Required Information
  • Full Name(s) and a valid photo identification
    For you and your spouse if apply
  • Social Security numbers or ITIN cards are REQUIRED
    For you, your spouse and any other persons listed on your tax return
  • Current address and phone number(s)
  • Accurate Date of Birth for all individuals listed on the return
  • Childcare provider(s) - name, address, tax payer ID
    If caliming the Child/Dependent Care Credit
  • Alimony Paid - name and Social Security number of individual paid
  • Proof of Account Number and Routing Number of your bank for a direct deposit of your refund into your checking and /or savings account
